Members Fee/ Fee-For-Service

•  Access- Members will have to the provider via phone, secure email, or text messaging. Why not take care of those needs that can be taken care of without leaving work or home. 
• Quality vs Quantity- A minimum of 30 minuets is scheduled for all follow up visits, new patients may take longer. Plus unlimited access through via text/email or Telehealth 

• With your monthly fee, you get follow-ups at no additional cost.
  • Office visits (wellnes, acute care, disease management.
  • Ability to communicate with the provider and staff via phone/text including appropriate after hours.
  • Coordination of care with specialist and other care providers.
  • Discounted Labs
  • Access to discounted medications 

The following are the differences between membership and insurance.

• Excellent care regardless of insurance
• Ability to answer questions and handle problems without requiring an office visit. 
• More time spent with the patient. 
• No waiting for office visits.
• Freedom from burdensome documentation rules.
• Advocating for the  patients so they get the best care from other doctors and avoid unnecessary and expensive testing. 
• Negotiate lower rates for labs and radiology tests for patients without insurance. 

• The membership fee covers only services rendered by the provider at Longevity, not the cost of visits to specialist or test that may be ordered. Insurance may still be used for those test or referrals ordered. 
• All patients, regardless of insurance status, pay the same a monthly "subscription" for their care and same services. No patient is turned away because of insurance, and no one is turned down for pre-existing  conditions. 

• For patients with insurance you may send bill to insurance by the pharmacy, labs, or radiology provider as usual. For those without insurance, labs are ran through Longevity at a discounted rate. Other orders such as prescriptions and other diagnostic testing Longevity will work to significantly reduce the cost.
